When I lived in New Orleans, admittedly a much smaller city than London or Paris, small-towners had the same reactions you two are describing.
They don't realize that big cities are really conglomerates of small towns and villages all stuck together, wherein different parts of the city can be radically different from each other. In that way, big cities can be quite provincial, with residents of one part of town sticking there with no desire to visit any other part.

I currently live in Mississauga, which has a population about 750,000. I find it distinct from Toronto. It's definitely a suburb, but it was a suburb created by amalgamating 6 little towns, so there are noticeably different feels to the various areas.
Overall, I like it. I was raised in a town of about 5,000 15 miles east of Montréal. Most people were either related to me or knew my parents. As a teenager, I swore I would get out of it as soon as I could. I've lived in Montréal, Vancouver and a few smaller places. I didn't like Vancouver, although I had a bad start and this was in 1974, when Vancouver was still very much a small town in mentality.
Would I move back to a small town? Sometimes I think I'd like to - I'd like to try living somewhere with about 5,000 people now. I do want some amenities - restaurants, library, some cultural life like local theatre or art gallery. At this point, with my employment and my mother's situation, where we are suits us.

Since I first piped in, we had a winter with practically no snow, so I did pretty much ride my bicycle all year long, and when there was a bit of snow, it was easy to walk as it was very mild.
Small town no, kerouac, though I could live somewhere like Perugia, as Rome is a couple of hours direct by train from city centre to city centre.

Yes, there are some medium-sized places where I could probably be happy, but I doubt anything under 50,000.

I live in Phnom Penh, Cambodia, with 1.5- 2 million inhabitants.
I can get nearly everything I need here.
