A second source familiar with the intelligence told CNN that the US has been closely monitoring reports on Kim's health. Kim recently missed the celebration of his grandfather's birthday on April 15, which raised speculation about his well-being. He had been seen four days before that at a government meeting.
Another US official told CNN Monday that the concerns about Kim's health are credible but the severity is hard to assess.
Daily NK, an online newspaper based in South Korea that focuses on North Korea, reports that Kim reportedly received a cardiovascular system procedure on April 12.
Kim received the cardiovascular system procedure because of "excessive smoking, obesity, and overwork," according to the news site, and is now receiving treatment in a villa in Hyangsan County following his procedure.

Just to add, I've been following this news since it first emerged.
What is interesting is that NBC news tweeted that Kim was brain dead, and then retracted the tweet:
The other interesting thing is that over the past few days, there has been an uptick in news stories about Kim's sister. Although a female leader of the DPRK is doubtful, this sister is ruthlessly competent.
There is speculation that he has Covid, among other things...so who knows? But what I do know is that the power vacuum this could create if he dies is scary... just in terms of the humanitarian crisis it could cause. With the pandemic having massive economic impacts on the rest of the world, other nations will be in no position to respond.

One point made in that article is the lack of denials he's had heart surgery. Just the denial that he is seriously ill.

BEIJING/SEOUL (Reuters) - China has dispatched a team to North Korea including medical experts to advise on North Korean leader Kim Jong Un, according to three people familiar with the situation.
The trip by the Chinese doctors and officials comes amid conflicting reports about the health of the North Korean leader. Reuters was unable to immediately determine what the trip by the Chinese team signaled in terms of Kim’s health.
A delegation led by a senior member of the Chinese Communist Party’s International Liaison Department left Beijing for North Korea on Thursday, two of the people said. The department is the main Chinese body dealing with neighbouring North Korea.
The sources declined to be identified given the sensitivity of the matter.
The Liaison Department could not be reached by Reuters for comment late on Friday. China’s foreign ministry did not immediately respond to a request for comment late on Friday. [More details at the link]

It's a doppelganger and you can see his sister keeping a close eye on him as she is possibly the official successor but it takes time for this to become public so there is a stopgap solution. As with his father and grandfather, it took quite a lot of time between them popping their clogs and it being announced.

I think their date can be believed since spy satellite photos can confirm on which day the crowd was there. And besides that, none of the supreme leaders was ever found out to have died before the regime announced it, even when they delayed the announcement. I think Western countries are just frustrated to still have a country that can hide a thing or two.

Twitter folks are really chattering about this being a possible ringer, but who knows? Twitterers are focusing on the teeth and also small freckle on the nose of the genuine article vs the ribbon cutter. If indeed he's dead/incapacitated, I believe it will be much harder to conceal that information now vs in the past just due to advances technology, surveillance and other intelligence gathering that has become more sophisticated since the last regime change.
